What are Focals services?

Focals services are designed to support people and families with practical, day-to-day assistance and guidance. Depending on your situation, this can include help to navigate systems, build skills, strengthen independence, and connect with the right services at the right time.

Why the transition to Achieve Australia matters

Transitions between providers can feel stressful — especially when you’ve worked hard to build trust and routines. The goal is to keep what’s working, reduce disruption, and make sure your supports continue smoothly under Achieve Australia.

How to keep your supports consistent

  • Ask for a clear handover summary (what supports you received, what goals you were working on, what’s been effective).

  • Confirm your current goals and priorities — and update them if your needs have changed.

  • Clarify who your key contacts are at Achieve Australia and how to reach them.

  • Request a simple schedule or plan for the first 4–6 weeks so you know what to expect.

A note on choice, control, and advocacy

You have the right to understand your supports, ask questions, and be involved in decisions. If something isn’t working, it’s okay to raise it early. Clear communication and documented goals help keep everyone accountable.
